Overview
To identify the reasons why the upgraded decontamination unit could not open to provide the full range of sterile services an supplies required by the Trust.
- To identify, and to take corrective actions, to secure safe, sound, compliant and reliable services and equipment.
- To clarify the overall contractual position and responsibilities regarding the building, engineering works and equipment.
- To clarify any design faults and / or professional errors.
- Project manage corrective works and commissioning to going operational.
CPA’s Approach
- To establish contractual details and professional appointment.
- To inspect the upgraded building, engineering services and specialist equipment and to list faulty building works none performing equipment and engineering services.
- Seek to get correction of all faults through the main contractor and design team
- Agree an overall strategy with the Trust to correct all faults and go operational and to take the necessary outcomes.
- Seek legal advice – contractual issues.
Findings
- Elements of the design, developed by the appointed design team, were faulty.
- The main contractor (engineering company) generally performed well.
- The building and specialist sub contractors performed poorly (flooring – slab – damp proof course failures. Suspended ceiling faults – unstable- cracking)
- The nominated specialist equipment suppliers performed poorly
Outcomes
- All faults were corrected to compliance standards.
- The specialist equipment was commissioned and proven following which the decontamination unit went operational.
- All faulty suspended ceilings were replaced free of charge.
- The main contractor paid substantial damages towards the end of the corrective work.
- The design team withdrew all claims for additional fees and assisted in the corrective works.
- The majority of the flooring was recovered and the damp proof membrane was re-installed.
